Well, some poisons can kill via stroke. So even if he was confirmed to have a stroke, it’s not enough to completely rule out foul play.
I think his son alleges that Stalin was killed but there is no evidence.

A leader of any country will have top quality medial care. Sometimes that just isn’t enough. Strokes can be anywhere from so mild that the person recovers fully, to causing serious lasting brain damage or death.

Probably just a stroke. It is still a small wonder he lived for as long considering all the accumulated hardships and stress in his life.
